Mental teeth: when to erupt?

The mental teeth are the third molars, which start to erupt between the ages of 17 and 25. Sometimes these teeth emerge without any discomfort, but some patients complain of swollen or painful gums, often because there is not enough room for the mental teeth to emerge or because the mental tooth is in an irregular position. It is recommended to rinse your mouth with salt water during the eruption of the wisdom teeth to relieve pain and prevent bacteria from forming. Alternatively, you can apply essential clove oil to a cotton swab and apply it to the sore area. The pain should eventually subside, but sometimes it happens that the newly formed mental teeth have to be extracted – this surgical intervention is often indicated when the mental teeth cannot erupt properly or are not in the correct position.

Crowding of mental teeth

If the mental teeth are unable to erupt properly, crowding is diagnosed – a dangerous situation as crowding can lead to infection, failure of adjacent teeth, gum disease or cyst formation. To avoid these problems, dentists recommend that the erupted teeth are removed before an X-ray is taken, which helps to determine the necessity of straightening the teeth.

RETINAL TEETH – A RELATIVELY COMMON PATHOLOGY OF THE MENTAL TEETH

This is the name given to impacted or unerupted mental teeth, which may be fully formed but never erupted. Dentists distinguish between two main degrees of mental tooth retention:

  • The tooth is as if lying flat (full retention): in this case, patients do not experience any symptoms – this pathology is often detected accidentally (for example, when X-rays are needed before certain procedures). However, sometimes this pathology also causes certain diseases (damage to adjacent teeth, cysts, etc.);

  • Incomplete mental teeth (partial retention) usually lead to various complications: bacterial plaque can build up in the gingival pockets around the teeth, which are impossible to brush off. This is what creates a favourable environment for the formation of caries and the development of gum disease. In the case of partial retention, the damage extends not only to the mental teeth but also to the adjacent teeth.

Why is it important to remove thinning teeth?

Dentists usually advise the removal of such teeth in the presence of this pathology, as otherwise it can lead to certain complications, the most common of which are:

  • Inflammation of the mental tooth: if the mental teeth are not fully erupted, a pocket of mucous membrane is formed underneath which accumulates food debris, leading to the formation of bacterial plaque, which is quite difficult to remove over time. Sooner or later, purulent inflammation can develop, which, if left untreated, can lead to serious health problems: the infection can spread to the neck and throat. Inflammation of the surrounding soft tissues is usually manifested by swollen gums in the molars, bad breath, intense pain (which can involve not only the mental tooth but also the ear and the temple), fever and headache. This inflammation is usually treated with medication, but if it recurs, it is recommended to remove the mental teeth;
  • Root damage to adjacent teeth: there are also cases where a malformed tooth rests on the root of an adjacent tooth, causing it to dissolve. If this process is advanced, an abscess can develop and even endocarditis can result;

  • Cysts: these are more dangerous than they may seem at first sight. Over time, they start to erode the adjacent bone, leading to serious inflammation;
  • Mucosal damage is often diagnosed when teeth are not properly erupted and continuously tear the mucosa, eventually leading to abrasions and wounds, making basic chewing of food much more difficult or even very painful.

When is the best time to have this procedure?

The removal of these teeth is usually considered when the pain in the mental tooth becomes unbearable, but dentists recommend not waiting until the situation is that bad. If the extraction of mental teeth is carried out on patients under 20 years of age, the chances of infections and complications are significantly lower, so if you think you will have to undergo this procedure sooner or later, it is recommended that you consult your dentist. Before carrying out this procedure, your dentist will be sure to tell you about the advantages and disadvantages of mental teeth straightening so that your decision will be much easier. Of course, mental teeth can also be extracted in older patients, but consider that this surgical intervention may be more complex and the recovery period longer.

Straightening a mental tooth

Local anaesthesia is usually applied before the surgery to numb the mouth area. Usually, the mental teeth are removed in about 20-30 minutes, but the numbness of the anaesthetised area can be felt for another 2-3 hours. If the case is very complex, the removal of the mental teeth may take longer – general anaesthesia rather than local anaesthesia may be used.

What should I do after having my mental teeth straightened?

This surgery may cause discomfort for the first three days. This is perfectly natural, but if the pain is very acute, painkillers may be prescribed. In some cases, after the straightening, it may be necessary to suture the wound – if this is necessary, the surgeon will need to remove the sutures after 1-2 days (there is no need to worry about pain – straightening a mental tooth is a simple procedure that takes just a couple of minutes). For the first 24 hours after braces, it is advisable to avoid physical exercise, smoking and alcoholic beverages, very hot drinks and soft foods. If you notice any swelling, use ice (wrap a bottle of frozen water in a towel and place it on the cheek). If you follow all the recommendations given by your doctor, you will definitely not experience unpleasant symptoms.

Most dentists agree that the mental teeth play a very minor functional role, which is why teeth straightening is more often prescribed than treatment for mental teeth. However, treatment is also possible, but it is important to appreciate that access to these teeth is much more difficult. If the damage to the mental teeth is very advanced, treatment is not always effective and patients are recommended to opt for tooth extraction.

Mental teeth are extracted if they are severely damaged by caries, if treatment is too difficult or even impossible, if the patient complains of recurrent inflammation, if the aim is to prevent the formation of cysts, etc. Although it is not always easy to opt for a tooth extraction procedure, consider the fact that the pain of the mental teeth may become simply unbearable and the complications may be too risky.

It is recommended to avoid any non-emergency dental procedures during pregnancy, especially during the first trimester and late third trimester. However, if there is an urgent need, tooth extraction can be performed safely, taking all precautions.

Protective teeth should be removed if they cause problems such as inflammation, infection, pain, or if there is too little room for them to grow, which can cause damage to other teeth.

It depends on individual circumstances. If the extracted tooth is in a prominent position, or if its absence can cause problems with chewing, it may be recommended to replace it with an implant or other orthodontic appliances.

Prevention is very important as it helps prevent tooth decay, gum disease and other oral health problems. Regular visits to the dentist and good oral hygiene can reduce the risk of complications and ensure long-term dental health.
